English If you intend to continue working in the GDS in Wales, then a few weeks before you are due to complete your training year you will be sent a Change of Status form for completion.
You should return the completed form together with confirmation of your Vocational Training Number.
Once received, your status will be changed from Foundation Dentist to Performer, and you will be fully included on the Dental Performers List in Wales.
Applying for Performers list entry in England as a current FD in Wales
If you are planning on working in England post-DFT, as part of your application to the England Performers List you will be required to evidence successful completion of DFT by providing your VT number – VT numbers are issued in the third week of August (you will receive an email from HEIW with your VT certificate).
